The short version

Melanotan 2 is a lab-made copy of a natural body signal that tells skin to make pigment. Inject it and the skin darkens with little or no sun, because the molecule switches on the same receptors that a tan would. The same molecule also acts on the brain, where it can cut hunger and, in men, trigger erections. That triple effect — tanning, appetite, sex drive — is why people seek it out, and it is also why the side effects are wide-ranging.

No health agency anywhere has approved Melanotan 2 for any use. The strong human evidence is thin: a handful of small early-stage studies in a few people. Most of what is known about appetite and sex comes from animal work. The reports of real trouble — prolonged painful erections, kidney and muscle injury, and new or changing moles — come from individual case reports, not trials. What the Melanotan 2 record actually establishes

Melanotan 2 (also written Melanotan II or MT-2) is a cyclic heptapeptide — a small ring of seven amino acids — engineered at the University of Arizona in the late 1980s as a more potent, longer-lasting version of alpha-MSH (alpha-melanocyte-stimulating hormone, the natural hormone that signals skin to make pigment) [3]. It is a non-selective agonist: it switches on all five melanocortin receptors, MC1R through MC5R [11]. That breadth is the whole story. MC1R activation in the skin drives pigment; MC4R activation in the brain drives appetite and erection. One molecule, several receptors, several effects.

The genuinely strong human data is small. In a placebo-controlled pilot Phase I study, subcutaneous Melanotan 2 increased pigmentation in 2 of 3 healthy men after only five low doses, with no UV exposure [1]. In a separate double-blind crossover study of 10 men with psychogenic erectile dysfunction, the peptide produced clinically apparent erections in 8 of 10, extending mean tip-rigidity duration to 38.0 minutes versus 3.0 minutes on placebo [2]. Those are real, citable, controlled findings — and they are also the entire controlled human record. There is no completed Phase II or Phase III trial for Melanotan 2 [1][2].

Why a 'reviews' site leads with mechanism

Search the compound and most pages either sell it or scold it. This one does neither. The most useful thing a reader can have is a clear map of the receptor cascade, because the cascade explains everything else: why the tan persists for weeks after the peptide has cleared (melanin synthesis continues downstream of the receptor) [9], why appetite drops (central MC4R signaling) [5], and why erections appear unprompted (the same central melanocortin pathway) [2].

The mechanism also marks the boundary. The receptor pharmacology is well characterized. The community-reported effects — a faster tan, blunted hunger, surging libido, but also nausea, flushing, and darkening moles — are consistent with that pharmacology, but they are anecdotes, not controlled measurements, and we label them as such on the effects page. The serious-event reports — priapism, rhabdomyolysis, renal infarction, melanoma — are real published case reports [4], but a case report describes an association, not an established rate.
